name, date of birth, and current clothing sizes. Include a daytime phone number. You may send as many
photos as you like (see below)

2. Include a business size (#10) envelope, or larger, that is self-addressed and stamped. Make sure the envelope is
large enough to return your photo(s) and that the postage is sufficient if there are several photos.
